My experience begins about a month ago with my brother who had a brain injury in the middle of this year. I was not happy with this location but unfortunately this was the only location that the state was willing to pay for my brother to live. I tried to communicate with the staff in regards to my brother's safety, and they told me they're not babysitters. My brother had an opioid addiction, and we wanted to make sure he was monitored properly and not trying to leave out to get any drugs, but unfortunately they stressed about patient rights.. Even though he has a brain injury and he can't make clear decisions and exercising poor judgment and I was POA activated. To make a long story short, they allowed my brother to leave on his own terms, and as a result, he came back with drugs and overdosed. I have confronted the staff about this and they tells me that he has rights and they're not going to babysit, even after I stress to them do not allow anyone to pick him up and let him leave the site without notifying me and his family but they did not do this. My brother could not walk very well, so they allowed him to walk by himself with his wheelchair and he stumbled and fall, and fell directly on his face, unattended, and knocked out his front row of teeth. Although, a doctor has written a report indicating that my brother needed 24-hour assistant care for feeding, for bathing, and for any other needs because he could not do it for himself. They took my brother and moved him over into an assisted living that was for people that was ambulatory and they could do for themselves. They took my brother and move him in a room way in the back where he could not be supervised and he had to yell for help. There was one person that had to assist nine residents in this building with one resident being bedridden. You have to ask yourself.. Why do they have a bedridden tenant in an independent living building? Although my brother was being paid for by the state of Wisconsin.. thousands of dollars.. They started to charge my brother rent and made him 100% responsible for his meals, cleaning products, and laundry.. And to remind you.. My brother cannot do for himself even after they received paperwork from the doctor. I have asked them numerous times to move him back into their CBRF building and they told me they will not because he overdosed twice and they did not want to jeopardize losing their license or have their business shut down so they moved him in the independent living. They promised they was going to provide him the same assistant care and they did not. They left my brother laying in soaked garments, blankets, and sheets. They allow my brother to give his TV to the drug man when the drug man came to pick up my brother. They was not feeding him, but they provided him up to 15 pills per day in one sitting without no meals on his stomach, nor did they check his sugar glucose levels due to them saying they lost his meter and they did not order him any insulin. The end result behind all of this, my brother passed at that location from a heart attack. I wanted to bring my brother home so bad and was in the process of having him removed from that facility due to the negligence, and the unprofessional environment of workers that are disgruntled, lack training, and very uncompassionate towards the families of the patients that reside there at that location. This location should be shut down and the owner should never have the priority to open up another facility to care for anyone.. not even themselves. I recommend anyone to find another location or take your loved ones home and work with your family and you all take care of your loved ones yourself. This is not the day and age to trust anyone outside of your family to care for your loved one or else you will be sorry and especially if you deal with staffs like Jefferson Crest.

This place is very unprofessional I wouldn't put my own elderly mother and here. The caregivers smoke and drink on job. Employees fight on job site in front of residents. They dont drug test no worker's.
The owner dont Even know the half that goes on in his company. The State and County need to investigate them. Within the last 2 years so many caregivers Quick because of the unprofessional environment. I wouldn't recommend no worker or resident there. Very sad
